Full story review: fixed a real main-quest blocker

Went through the whole story end to end looking for gaps and inconsistencies. Found and fixed a real one: ten main story quests, spanning a big stretch of the mid-game, were quietly requiring a rare enemy drop to progress at all — if you never got the drop, or sold it without knowing it mattered, you could get stuck unable to continue. That's fixed now; those quests just progress normally. Also fixed Herath being misgendered in a bunch of older quest text, and a couple of smaller housekeeping items. Nothing else needed changing — the rest of the story held up clean.
